The British Medical Ultrasound Society (BMUS) has released the fourth edition of its Incidental Findings on General Medical Ultrasound Examinations guidance, marking the most significant revision since 2020. The 2026 update reflects advances in ultrasound technology, evolving clinical evidence, and the increasing role ultrasound plays in frontline diagnostics.

The guidance emphasises distinguishing clinically important abnormalities from normal variants to ensure proportionate, evidence‑based follow‑up while avoiding unnecessary patient anxiety or use of healthcare resources. For the first time, each recommendation includes an explicit evidence‑grading to improve clarity and support clinical decision‑making.

Key updates include revised pathways for liver lesions in chronic disease (aligned with LI‑RADS), reinforcement that gallbladder polyps ≤5 mm typically need no follow‑up, updated management of complex renal cysts incorporating contrast‑enhanced ultrasound, and refreshed ovarian cyst recommendations consistent with RCOG guidance. The document also reiterates that abdominal aortic aneurysms ≥3 cm should be clearly reported, with CT used when thoracic extension is a concern.

Overall, the 2026 edition provides a structured but adaptable framework to support safe, consistent, and patient‑centred management of incidental ultrasound findings across UK practice.
